If you are an active AAFPRS member and interested in the humanitarian efforts supported by the AAFPRS Foundation, please complete and submit the below Physician Interest Form. Your application will be reviewed, and a member of the AAFPRS Foundation will contact you with instructions on how to proceed.
This form must be completed and submitted by each physician interested in participating in any of the three FACE TO FACE (FTF) programs. Please be sure to complete the Physician Information along with the appropriate tab for the program(s) for which you are interested in participating, i.e. FTF International, FTF Faces of Honor or FTF Domestic Violence.

International Application for Sanctioned FTF Mission
Please complete this form if you wish to have your trip sanctioned as a FTF mission. The application will be reviewed by the FTF committee chair and FTF committee as outlined in the FTF Physician Guide. A Log In ID and Password will be provided to you once your mission is sanctioned which will provide you access to the FTF database for your specific mission.
International Application for Financial Stipend
For sanctioned FTF missions, a physician may apply for a financial stipend to offset costs for the trip. You must submit this form prior to the trip for approval. At the completion of the mission, submit receipts to the FTF Administrator in order to receive your financial support.
International Patient Data Sheet
A Patient Data Sheet should be established for each patient screened. This information is retained in the FTF database. Contact the FTF Administrator to discuss this form.

A Patient Visit Notes should be established for each patient screened. This information is retained in the FTF database. Contact the FTF Administrator to discuss this form.
